The Patient Review Instrument and accompanying SCREEN, is an assessment of a person’s overall health, and a measurement tool for their placement into a skilled nursing facility.

Cindy Hoffman, RN
Cindy Hoffman has 40+ experience as a Registered Nurse, and has recently spent over 20 years as an Admissions Coordinator in a 362 bed skilled nursing facility and has been completing PRI/SCREENS for over 30 years. Cindy has also worked as a Discharged Planner and Case Manager, in area hospitals. She is NYS PRI & SCREEN Certified, and can provide you with the knowledge and expertise you need to be sure your loved ones get the correct guidance for nursing home care.
